Service Description

The Foundation package is best for athletes who are looking to understand and optimize their biomarkers plus have more maintenance throughout the year. The Foundation package consists of two comprehensive Performance Consults. That's a total of four appointments at a $600 value! Please schedule your first appointment online. The next three appointments will be scheduled after first visit with Sam. Similar to a physical, the Performance Consult is a comprehensive exam which evaluates an athletes medical history, general well-being, medications, current diagnoses plus a head-to-toe assessment. Additionally, we will look deeply at your training, sleep, recovery modalities, nutrition, and mental well-being as it relates to sport. A plan of care will be developed which includes lab orders plus a follow-up appointment to discuss the lab results, potential referrals and a suggested performance plan that you/we can discuss with your coaches, nutritionist, family and other care providers. *This is a cash-based service. Sam is happy to provide a bill for you to send to your insurance for reimbursement. **Lab fees are separate costs that will be managed between you, your insurance company and the lab. Even though Sam is considered an out-of-network provider, insurance will cover your lab fees per your insurance policy, as the lab will be the entity billing your insurance for lab fees, not Athlete Health & Performance. Typically, health insurance will cover 100% of all lab fees once every year during an annual physical.
